Why do I get an error when I enter the unlock code for my Nokia 1616-2?

I am trying to unlock my phone. I entered the generated code to my Nokia 1616-2 IMEI 355369049864259 but it says “not allowed” every time I enter the code. What should I do? Also, how can I tell which country the phone was manufactured or which network was used first.
